Anglican Record Request Form

This request is for a Baptism, Marriage, or Burial entry in an Anglican Parish register located in the Diocese of Niagara. For all other dioceses or religions, please contact the appropriate archive.

You will receive a pdf copy of the page from the Parish register. We do not have the actual certificate of baptism, marriage, or burial.

Due to the volume of requests, we can only provide a copy for requests that include the name, parish, and date within 5 years of the event in question.

Year of the event. Please include full date, if known. We will check roughly five years on either side of the given date.

Registers cover specific dates and events, by checking the listing for the church you can identify which register covers the date you need.
