Send your equipment requests using the form below, or email/fax all required information to equipbkg@mcmaster.ca / fax 905-526-6063.

Requests should include your name, address and phone number, department, course number, date/time required, location required, and equipment required.

You can also call by phone 905-525-9140, ext.22764 David Kidney, Manager Classroom Audio Visual Services. Our office is located in Mills Memorial Library, L118.

All fields must be completed before submitting this form. Equipment must be booked by 3pm, business day prior, to actual requested booking date
